Transcript Request Form

If you would like an unofficial ELI transcript immediately, you can download one yourself! Just go to UDSIS Student Center under Grades and Transcripts.

Students must pay their bills before they can download an unofficial transcript.

Important Notes

Transcript requests may take up to 3 business days to process.
Fees: US $5 for each paper transcript (both official and unofficial). Official and unofficial electronic transcripts are free.
- You can ask us to send your transcript by express mail (ex: DHL, FedEx). This costs an additional $65.

We highly recommend that international shipments be sent via express mail. It allows the ELI to track the package, and it is much faster.

- To pay, please use the Secure Payment form online.
If you owe money, you need to pay before you can receive your transcript.
If you need to send your transcript to more than one person, please fill out a separate Transcript Request Form for each recipient.

Please write the first and last name of the person who will pick up your transcript at the front desk at 108 E. Main St. This person must show photo ID to pick up your transcript.

Be sure to bring photo ID! You will need it to pick up your transcript.

How many transcripts would you like to order? (Note - If you want transcripts to be sent to multiple places, you must complete a separate Transcript Request Form for each recipient.)
